Community Nursing Services has partnered with Davis County Schools to offer flu immunization clinics to the public.  CNS will give $2 back to the school for every flu immunization given at this event. 

Cost is $25 for a flu shot or $30 for flu mist.  CNS is a preferred provider and is contracted to directly bill the following insurances:  Aetna, Altius, Blue Cross, DMBA, Educators Mutual, PEHP, Select Health, Summit Care, United Health Care, Medicare Part B, and some Medicare Advantage Plans.  The policy holder is responsible to determine if flu immunizations are covered under their specific insurance plan.  Insurance card is required to bill insurance.  If a co-payment is required, one may be collected at time of service.

Flu Clinics will be held from 4:00-7:00 p.m. on:

October 4   - Kaysville Jr. High
October 24 - Burton Elementary
October 24 - Columbia Elementary
October 24 - Kaysville Elementary
October 24 - Snow Horse Elementary
October 24 - Windridge Elementary
